If you would like to reserve a spot simply submit the following forms with payment.  Please submit one "Registration" form and "Waiver" form per child. Please use separate forms for siblings.

If registering for a Lil' Flipper class, the parent and any adult assisting the child in class, is required to complete the adult waiver located on the reverse side of the student waiver.
